If you are started on insulin, let your doctor know. Pregnant women become diabetic in pregnancy because their placenta is making a hormone that makes it harder for them to metabolize sugar. If you find that you are on a certain dose of insulin and then the dose needs to be decreased, this could be a sign that the placenta is starting to get old and you should let your doctor know. It is okay if the insulin requirements keep going up.

What Is The Difference Between The 1 Hour Challenge And The 3 Hour Oral Glucose Tolerance Test

The 1-hour test only requires one blood sample after drinking 50 grams of glucose, whereas the 3-hour test requires a fasting blood draw, and 3 additional blood draws after drinking 100 grams of glucose.

The 1-hour test is a screening test, while the 3-hour test is a confirmatory test.

If you test positive on the 1-hour screening test, you will then be asked to undergo the 3-hour confirmatory glucose tolerance test.

What Changes They Have Made To The A1c Guidelines

For most adults, the American Diabetes Association currently recommends an A1C of less than 7%, a TIR of over 70%, and a time below the range of less than 4%. In past years, the Standards of Care contained an A1C Testing section that advised diabetics to test their A1C two to four times a year with an A1C target of less than 7%.

What Does The Blood Glucose Test Involve

All pregnant women are offered this screening test between 24-28 weeks gestation. When other risk factors are present, such as obesity, the test may be offered earlier and then repeated if initially normal. The screening test for GD is called a glucose challenge, which aims to see how your body is handling sugars. The measurement is taken 1 hour after you consume a glucose drink. This test can be conducted at any time of day as it is not a fasting test. If your blood sugar is normal after the challenge, you will not require any more testing. If it is high, you will have a second test. This test is done in the morning while you are fasting and will require you not to eat prior to the test. You will have your glucose tested before consuming a glucose drink and then tested again 1 and 2 hours later. If your glucose is higher than specific cut-off values, it means you have GD. Why The Test Is Performed

This test checks for gestational diabetes. Most pregnant women have a glucose screening test between 24 and 28 weeks of pregnancy. The test may be done earlier if you have a high glucose level in your urine during your routine prenatal visits, or if you have a high risk for diabetes.

Women who have a low risk for diabetes may not have the screening test. To be low-risk, all of these statements must be true:

• You have never had a test that showed your blood glucose was higher than normal.
• Your ethnic group has a low risk for diabetes.
• You do not have any first-degree relatives with diabetes.
• You are younger than 25 years old and have a normal weight.
• You have not had any bad outcomes during an earlier pregnancy.

How To Prepare 3 Hour Test

If your doctor would like to see additional testing, based on your results from the 1 hour test, you may be asked to have a 3 hour OGTT. It is important to follow all of the instructions very carefully, as it will affect the results of the test.

When you arrive at the office a sample or baseline glucose test will be run and used for comparing other glucose values. You will be asked to drink a sweet liquid containing a measured amount of glucose, it is best to drink it quickly. Then blood samples will be collected at timed intervals of 1,2, and 3 hours after you drink the glucose.

Can I Do A Fasting Blood Sugar Test At Home Instead

There are kits that allow you to test your blood sugar at home. But they should not be used to diagnose diabetes.

The results are often not as accurate as tests by a healthcare provider. Several factors can affect accuracy, such as:

• Environmental conditions.
• Unclean skin on the finger being pricked.
• Expired or damaged test strips.
• Not enough blood in the testing sample.

Delivery When You Have Gestational Diabetes

When planning for the baby’s arrival, the doctor will access the size of the baby to determine if you can deliver vaginally. If you have been able to keep your blood glucose well-controlled, your baby’s weight is within an appropriate range, you don’t have any other pregnancy concerns such as high blood pressure, and you are not on medication, than your labor should proceed just as it would if you didn’t have gestational diabetes. Of course, your delivery team will monitor your blood sugar throughout.

If your baby is considered too large for you to deliver vaginally then you will likely be induced at week 38 or 39. You and your doctor may also decide that a cesarean section may be a better route for delivering the baby. Should blood sugar levels get too high during labor, the baby may release more insulin in response. That increases the risk of the baby developing low blood glucose after birth so a C-section may be considered.

Do I Need To Be Fasting For The Pregnancy Glucose Test

You do not need to be fasting for the traditional 1 hour test.

However, there is a 2 hour 75-gram glucose test that can be done.

For the 75-gram oral glucose tolerance test, you need to be fasting, and you have to have your blood sugar level taken three times.

The first blood test will look at your fasting glucose, then you will get two more blood tests done. Once after an hour, and again after 2 hours.

Most OBGYN providers use the 1-hour test.

What Does The Glucose Tolerance Test Mean For Me After Baby

Most often all the issues with blood sugar go away after the baby is born.

Many hospitals will check your blood sugar the next day or so to see how it is doing, but likely you will be able to eat whatever you want.

However, it does show that you are an increased risk of diabetes later in life, so consider making sure that you diet balances carbs and proteins, and make sure youre exercising to mitigate that risk.

What Does It Involve

In the challenge test, you drink a glass of water with 50 grams of sugar dissolved in it. One hour later, blood is taken from a vein in your arm to measure your blood sugar levels. If the value is below 7.5 millimoles per liter , the results are considered to be normal and no more tests are done.

If the results of the challenge test are abnormal, a glucose tolerance test is done. Unlike the challenge test, it has to be done on an empty stomach. In other words, you shouldnt eat anything for at least eight hours beforehand. You’re only allowed to drink still mineral water or tap water during this time. At the beginning of the test, a blood sample is taken. You then drink a sugar solution containing 75 grams of glucose dissolved in 300 ml of water. A blood sample is taken from a vein in the arm one hour later, and then again after two hours. If any of the following three blood sugar levels is reached or exceeded, gestational diabetes is diagnosed:

• After fasting: 5.1 mmol/l
• After one hour: 10.0 mmol/l
• After two hours: 8.5 mmol/l

The test results are documented in your maternity records. In Germany, the costs of the glucose challenge and tolerance tests are covered by statutory health insurers.

There are no known risks to the baby. Some women find the sugar solution unpleasantly sweet.

What Does The Glucose Tolerance Test Measure

The classic oral glucose tolerance test measures blood glucose levels five times over a period of three hours. Some physicians simply take a baseline blood sample followed by a sample two hours after drinking the glucose solution. In a person without diabetes, the glucose levels rise and then fall quickly. In someone with diabetes, glucose levels rise higher than normal and fail to come back down as fast.

People with glucose levels between normal and diabetic levels have so-called impaired glucose tolerance . People with impaired glucose tolerance do not have diabetes.

Each year, 5% to 10% of people whose test results show impaired glucose tolerance actually develop diabetes. Weight loss and exercise may help people with impaired glucose tolerance return their glucose levels to normal. In addition, some physicians advocate the use of medications, such as metformin , to help prevent/delay the onset of overt diabetes. Studies have shown that impaired glucose tolerance itself may be a risk factor for the development of heart disease, and whether impaired glucose tolerance turns out to be an entity that deserves treatment itself is something that physicians are currently debating.

How Gestational Diabetes Can Affect Your Pregnancy

• your baby developing low blood sugar or yellowing of the skin and eyes after he or she is born, which may require treatment in hospital

Can Gestational Diabetes Be Prevented

Its not possible to prevent gestational diabetes entirely. However, adopting health-promoting habits can help reduce your chances of developing the condition.

If youre pregnant and have one of the risk factors for gestational diabetes, try to eat a nutritious diet and get regular exercise. Even light activity, such as walking, may be beneficial.

If youre planning to become pregnant in the near future and youre overweight, consider preparing for your pregnancy by talking with a healthcare professional about ways to safely lose excess weight.

They can help you create a plan to reach and maintain a moderate weight. Even losing a small amount of weight can help you reduce your risk of gestational diabetes.
